


Radha Devi, MD, Timothy Evans, MD, and Rob Young, MD, have been spearheading the ADEPT (Achieving Diagnostic Excellence Through Prevention and Teamwork) initiative at WashU in collaboration with Colorado’s Institute for Healthcare Quality, Safety, and Efficiency (IHQSE). Along with 14 other academic medical institutions across the United States, the WashU ADEPT team is leading the charge to reduce the burden of diagnostic error.

As part of these ongoing efforts, these academic centers are working together to launch the Diagnostic Cross-Check initiative on October 6, 2025. In preparation for the launch of this initiative, doctors Radha Devi, Timothy Evans, and Rob Young attended the Diagnostic Excellence Capacity Building Program at IHQSE in July. This 3-day session focused on evaluating critical leadership skills and exploring methodologies for the successful launch and implementation of the Diagnostic Cross-Check.
The Diagnostic Cross-Check aims to target the diagnostic delay that medical inpatients experience. The ADEPT collaborative found that 1 in 13 medical inpatients could have had an accurate diagnosis sooner. This initiative will focus on both reducing the rate of diagnostic error and avoiding diagnostic delays.

“Work done by the ADEPT collaborative shows that the rate of diagnostic delay in 14 academic medical centers is 8%. In just 8 weeks, this group of 14 institutions from across the country will launch a new initiative called the Diagnostic Cross-check, which will help reduce the burden of diagnostic error in their hospitals.”
